I have read and agree to the terms and conditions of this page as follows: Coaching Agreements and Waivers for Sound Mind Life Coaching and Dr. Trashonda Mizuno Malbrough: The Client/Coach relationship is a powerful one! It is our mission to inspire lasting, meaningful transformation. Coaching Sessions Procedures The Client will call The Coach at the phone numbers designated by The Coach. Although we make efforts to remind you to book your sessions the Client is responsible for booking and attending his/her sessions. The Client will pay for their own telephone and technology charges. Appointment Cancellation, No-Shows, Client’s Failure to Schedule: If you cancel a 1:1 session within the 24 hour cancellation policy, you have 30 days to make up your session. Cancellations must be made 24 hours in advance for phone 1:1 sessions unless there is a true emergency. To cancel, call 805-758-8199 or email [email protected], or cancel via the calender link in your confirmation email. The Company is not obligated to give any refund (full or partial) or time extension to schedule or store up (bank) sessions and services for later use to a client who does not schedule or attend their sessions or services (including 1:1 sessions, couples’s sessions, group sessions, or otherwise) they have enrolled in within the month they were to use their sessions or services. Packages In the event that the Client wants to upgrade or downgrade to a different package that involves 1:1 sessions, couple’s sessions, or group sessions, the Client must give The Coach a 30-day notice in writing to [email protected]. Text messages, social media direct messages, and other forms of messaging will not be considered “written notice”. If the Client desires to change their monthly coaching package within the 30 days, it may be denied, as the auto-billing cycle will not allow it. Client Commitments We are HONORED to only coach Clients who are coachable, committed to their results, and mentally fit . The Client is responsible for their results. The Coach’s mission is to be a vehicle of transformation, not a crutch or a quick fix. If the Coach believes the Client and Coach are not the best match, the Coach will professionally inform the client with care and professionalism. However, the Coach will not tolerate abuse, inappropriate behavior, misuse, insulting language, threatening language or the like. Such behavior may result in an immediate end of Coaching Services and may not include a refund or any further obligation on the part of the Coach or the Company. Release & Waivers Dr. T MM & Sound Mind Life Coaching offers each client the utmost in care, confidentiality, and coaching. This agreement is not a guarantee of results, as results vary for each client. It is the Client's responsibility to book and attend the sessions and do the recommended work. We strongly encourage each client to embrace the coaching experience. NATURE OF RELATIONSHIP: The Coaches may have backgrounds in: Behavioral Analysis, Behavioral and Cognitive Change, Emotional Freedom Technique, Sales Training, Time Management, Values Clarification, Motivation, Neuro-Linguistic Programming, Health, Trauma Healing, Grief Counseling, Marketing, Leadership, Strategic Business Planning, Entrepreneurship, Spiritual Leadership, Corporate and Human Resources Strategies, Goal Setting, Advanced Life-Coaching Strategies and other disciplines. CLIENT HAS BEEN MADE AWARE THAT THE COACHING- CONSULTING RELATIONSHIP IS IN NO WAY TO BE CONSTRUED AS PSYCHOLOGICAL COUNSELING OR ANY TYPE OF THERAPY. In the event the client feels the need for professional psychological therapy or psychiatric services it is the responsibility of the client to seek that type of licensed professional. The Client enters into this agreement with the understanding that they are responsible for creating their own results. The Client also agrees not to hold The Coaches or Dr. Trashonda Mizuno Malbrough or Sound Mind Life Coaching, et al liable for any actions or results for adverse situations created as a direct result of the coaching or any specific referral given by The Coaches or Dr. Trashonda Mizuno Malbrough or Sound Mind Life Coaching, et al. The Client agrees that their physical, mental, emotional, spiritual, family related, career related, and life choice related health is their sole responsibility. Any legal actions must be conducted in the state of California. This waiver applies to previous and future Coaching and Consulting Conversations AND Materials from Trashonda Mizuno Malbrough, Sound Mind Life Coaching, et al.
